Job Title: APN - Ventricular Assist Device Coordinator

At Deborah Heart and Lung Center, we are seeking a highly skilled and dedicated Advanced Practice Nurse to join our team as a Ventricular Assist Device (VAD) Coordinator. This is a unique opportunity to work with a high-risk patient population and make a meaningful impact on their lives.

Job Summary:

The VAD Coordinator will be responsible for overseeing the development, implementation, and maintenance of all aspects of care for patients with a ventricular assist device. This includes coordinating and managing patient care throughout the continuum, optimizing clinical, functional, and psychosocial status, and providing leadership and mentorship to the Cardiologists and Nursing Department.

Responsibilities:
  • Develop and implement care plans for patients with VADs
  • Coordinate with the Heart Failure Medical Director to ensure optimal patient outcomes
  • Provide education and support to patients and their families
  • Collaborate with the multidisciplinary healthcare team to ensure seamless care coordination
  • Participate in performance improvement initiatives to enhance patient outcomes
Requirements:
  • Master's degree or greater in Nursing
  • Minimum 3-5 years of nursing experience, preferably in CV post-surgical management
  • New Jersey License as a Registered Nurse and Certification as an Advanced Practice Nurse
  • Current CDS and DEA required within 6 months of employment
