Job Overview

POSITION SUMMARY

We are seeking a dedicated individual to join our team as a Case Manager. This role is essential in delivering community-oriented case management and comprehensive support services to eligible members facing multiple chronic health challenges. The successful candidate will engage in various responsibilities, including:

  • Providing tailored case management services to clients.
  • Identifying and connecting eligible individuals to appropriate project services.
  • Participating in outreach initiatives at community locations and within our service facilities.
  • Offering follow-up support to encourage engagement with potential participants or those who may have disengaged from care.

Prior experience in case management is preferred, particularly with individuals experiencing mental health issues and substance use disorders. Flexibility to work evenings and weekends is required.

SALARY RANGE:

$29.50 per hour, dependent on experience.

Bi-lingual proficiency in Spanish and English is preferred.

A Bachelor’s degree in social work or a related field is required, or at least one year of relevant case management experience.

8 hours per day, 5 days per week.

KEY RESPONSIBILITIES:

  1. Maintain professionalism in dress, language, and behavior.
  2. Support and adhere to the mission and objectives of the organization.
  3. Understand and uphold professional ethics and policies.
  4. Inform clients about additional services available that may benefit their families.
  5. Participate in training sessions and educational workshops.
  6. Attend all mandatory staff meetings.
  7. Collaborate effectively with colleagues regarding client progress and activities.
  8. Work closely with multidisciplinary team members to create comprehensive Individualized Treatment Plans for assigned clients.
  9. Assist clients with employment and vocational referrals, including resume writing and interview preparation.
  10. Provide outreach and follow-up services to clients, ensuring continued engagement and support.
  11. Facilitate discharge planning from the initial admission, addressing housing, employment, medical, psychiatric, financial, and aftercare needs.
  12. Establish direct communication with both internal and external entities to assist clients in resolving various issues.
  13. Document client progress and case management activities thoroughly in their Medical Records.
  14. Perform additional duties as assigned by supervisory staff.
