Supportive Services Case Manager

POSITION SUMMARY
We are looking for a dedicated individual to join our team as a Case Manager. This role is essential in delivering comprehensive, community-oriented case management and supportive services to eligible Medi-Cal beneficiaries facing multiple chronic health challenges. The Case Manager will engage in various responsibilities, including:
- Providing tailored case management services to clients.
- Identifying and connecting eligible individuals to necessary project services.
- Participating in outreach initiatives at community locations and within our service facilities.
- Conducting follow-ups to re-engage potential clients or those who may have disengaged from care.
QUALIFICATIONS
Preferred candidates will have prior experience in case management, particularly with individuals dealing with mental health issues and substance use disorders. Availability for evening and weekend shifts is required.
SALARY RANGE:
$29.50 per hour, dependent on experience.
ADDITIONAL REQUIREMENTS:
Bi-lingual proficiency in Spanish and English is preferred. A Bachelor’s degree in social work or a related field is mandatory, or alternatively, one year of relevant case management experience.

WORK SCHEDULE:
Full-time position requiring 8 hours per day, 5 days per week.
KEY RESPONSIBILITIES:
- Maintain professionalism in appearance, communication, and conduct.
- Demonstrate a thorough understanding of and commitment to the Agency's mission and objectives.
- Adhere to professional ethics and policies.
- Inform clients about additional services available that may benefit their families.
- Participate in ongoing training and educational opportunities.
- Attend all mandatory staff meetings.
- Collaborate effectively with colleagues regarding client progress and activities.
- Work alongside multidisciplinary team members to create individualized treatment plans for clients.
- Assist clients with vocational referrals and provide guidance on job readiness.
- Conduct outreach and follow-up services to support clients in and out of treatment.
- Facilitate discharge planning from the point of admission, addressing various client needs.
- Engage with internal and external resources to assist clients in resolving various issues.
- Document comprehensive progress notes in clients' medical records, tracking their journey and engagement.
- Perform additional duties as assigned by supervisory staff.
