Position Summary:
The Supportive Services Specialist is integral in linking individuals and families to essential resources and services that facilitate self-sufficiency and elevate their quality of life. This role encompasses direct client engagement, resource management, and community outreach to guarantee that clients receive holistic and impactful support.
Key Responsibilities:
Client Assessment and Support:
- Conduct thorough assessments to determine the needs and strengths of clients.
- Collaborate with clients to create personalized service plans that address their specific needs and aspirations.
- Provide continuous support and follow-up to ensure clients are progressing towards their objectives.
- Advocate for clients to access vital services, resources, and benefits.
- Uphold confidentiality and maintain the highest standards of client care.
- Connect clients with suitable services and resources, including healthcare, housing, employment, education, and social services.
- Keep an updated database of community resources and service providers.
- Establish and nurture relationships with community partners, agencies, and service providers.
- Represent EHCEC at community events and forums to advocate for our supportive services.
- Conduct outreach and educational initiatives to raise awareness of available support services.
- Maintain accurate client records, documenting services provided, client progress, and outcomes.
- Prepare reports on client metrics and success stories for stakeholders.
- Utilize data to enhance service delivery and inform strategic planning.
- Bachelor's Degree in Social Work, Sociology, Psychology, or a related field preferred; equivalent experience may be considered.
- At least 2 years of experience in social work, case management, or a related field.
- Fluency in English & Spanish preferred.
- Familiarity with local social service, public health, and nonprofit organizations.
- Understanding of social work principles, ethics, and best practices.
- Ability to present a friendly and positive demeanor to clients, staff, and the public.
- Strong organizational and project management skills.
- Flexibility to adapt to changing circumstances.
- Willingness to work flexible hours, including evenings and weekends as required.
- Valid driver's license and reliable transportation.
- Perform additional duties as assigned.
- Experience working with diverse populations and addressing the needs of marginalized communities.
- Strong advocacy and client support skills.
- Proficiency in data management and reporting.
- Ability to prioritize work and devise solutions to work-related challenges.
- Cultural competence and sensitivity to the diverse backgrounds of clients.
- Ability to remain composed in challenging situations.
The staff member must be available for meetings during evenings and weekends as necessary. EHCEC promotes a collaborative work environment where employees support each other in their responsibilities.
