Position Summary:
The Supportive Services Specialist is integral in linking individuals and families to essential resources and services that facilitate self-sufficiency and elevate their quality of life. This role encompasses direct client engagement, resource management, and community outreach to guarantee that clients receive holistic and impactful support.

Key Responsibilities:
Client Assessment and Support:

  • Conduct thorough assessments to determine the needs and strengths of clients.
  • Collaborate with clients to create personalized service plans that address their specific needs and aspirations.
  • Provide continuous support and follow-up to ensure clients are progressing towards their objectives.
  • Advocate for clients to access vital services, resources, and benefits.
  • Uphold confidentiality and maintain the highest standards of client care.
Resource Coordination:
  • Connect clients with suitable services and resources, including healthcare, housing, employment, education, and social services.
  • Keep an updated database of community resources and service providers.
Community Engagement:
  • Establish and nurture relationships with community partners, agencies, and service providers.
  • Represent EHCEC at community events and forums to advocate for our supportive services.
  • Conduct outreach and educational initiatives to raise awareness of available support services.
Data Management and Reporting:
  • Maintain accurate client records, documenting services provided, client progress, and outcomes.
  • Prepare reports on client metrics and success stories for stakeholders.
  • Utilize data to enhance service delivery and inform strategic planning.
Required Skills & Abilities:
  • Bachelor's Degree in Social Work, Sociology, Psychology, or a related field preferred; equivalent experience may be considered.
  • At least 2 years of experience in social work, case management, or a related field.
  • Fluency in English & Spanish preferred.
  • Familiarity with local social service, public health, and nonprofit organizations.
  • Understanding of social work principles, ethics, and best practices.
  • Ability to present a friendly and positive demeanor to clients, staff, and the public.
  • Strong organizational and project management skills.
  • Flexibility to adapt to changing circumstances.
  • Willingness to work flexible hours, including evenings and weekends as required.
  • Valid driver's license and reliable transportation.
  • Perform additional duties as assigned.
