Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team in providing high-quality patient care in the comfort of their own homes.

About the Role

The Licensed Practical Nurse (LPN) will be responsible for delivering technical skilled care to patients in their homes, in accordance with the plan of care and in coordination with other members of the healthcare team.

Responsibilities
  • Provide quality patient care and perform technical skilled care in the patient's home
  • Instruct the patient's family in the care of the patient and maintenance of a healthy environment
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily
Requirements
  • Current LPN licensure in NH
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification required
