Job Summary

We are seeking a compassionate and skilled Home Care Hospice Aide to provide personal care and home management tasks to patients in their residences, under the supervision of a RN Case Manager.

Key Responsibilities
  • Deliver high-quality patient care, following the nurse aide care plan, in areas of personal hygiene, elimination, feeding, and mobility.
  • Report changes in patient condition, concerns, and participate in interdisciplinary team conferences as necessary.
  • Collaborate with the IDG and other departments to achieve patient care goals.
  • Document patient care according to agency policy and procedures.
  • Manage time, people, medical supplies, and technology professionally.
  • Establish and adhere to a timeframe for patient visits.
  • Maintain a clean, safe, and comfortable environment for patients.
  • Teach and reinforce home safety, energy conservation, and sanitation.
  • Perform light housekeeping tasks and assist with meal preparation.
  • Demonstrate excellent communication skills and build successful relationships with patients and families.
  • Comply with organizational nurse aide standards and participate in staff training and education programs.
