We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services to patients in the High Point, NC area.
Key Responsibilities- Provide safe and individualized patient care, following established care plans and protocols.
- Monitor patient vital signs and report any abnormalities to the Case Manager and Physician.
- Administer medications, feedings, and other treatments as needed, and document the results.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients and their families.
- Ensure infection control policies are maintained and report any potential communicable diseases or infections.
- LPN/LPT/LVN degree from an accredited college of nursing, with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications and providing home health care services.
- Ability to lift and carry items weighing up to 20 pounds, and to bend, stoop, push, pull, sit, and walk for periods of time.
