Position Overview

Essential Responsibilities:

  • Perform tasks under the guidance of a registered nurse.
  • Engage in evaluating the patient's physical and psychological well-being, as well as their responses to medical conditions and treatment plans.
  • Monitor the patient's status and communicate any changes or needs for nursing interventions to the nurse case manager or physician as instructed by the RN.
  • Conduct nursing visits and execute care activities aligned with the patient's individualized Plan of Care established by the RN. This includes administering prescribed treatments, managing wounds, performing phlebotomy, dispensing medications, and providing fundamental bedside care. Additionally, responsibilities may include irrigating, collecting blood samples, changing dressings, and administering medications/fluids through various access points including peripheral, PICC, midline, central venous access, and IVADs. May also involve changing dressings and sites, and administering medications/fluids through subcutaneous access catheters, as well as removing specified catheters as directed by the RN.
  • May have the authority to pronounce death.
  • Act professionally to collaborate with physicians, healthcare providers, and interdisciplinary team members, ensuring timely communication of relevant information to appropriate healthcare professionals.
  • Document assessment data and interventions accurately and promptly in accordance with agency nursing standards.
  • Participate in interdisciplinary team meetings as directed by management.
  • Delegate suitable personal care responsibilities to Hospice Aides and oversee the completion of these tasks.
  • Assist in basic nursing and personal care as needed.
  • Engage in educating patients and caregivers on home care practices as assigned by the staff nurse.
  • Support audit processes as assigned.
  • Participate in the nursing on-call schedule as required, under the supervision of the RN on call.
  • Adhere to agency nursing standards, ensuring timely documentation and demonstrating proficiency in approved infusion therapy tasks.
  • Physical capability to lift, turn, and transfer patients; minimum lifting requirement of 40 pounds.
