Position Overview

Essential Responsibilities:

  • Functions under the guidance of a registered nurse, executing tasks as assigned.
  • Engages in evaluating the patient's overall health, including both physical and psychological aspects, as well as their responses to medical treatments.
  • Monitors the patient's status and communicates any significant changes or needs for further nursing interventions to the designated nurse case manager or physician as instructed by the RN.
  • Conducts nursing visits and carries out nursing procedures in alignment with the patient's individualized Plan of Care established by the registered nurse. This encompasses administering prescribed medical treatments, managing wounds, performing phlebotomy, dispensing medications, and providing essential bedside care. Additionally, may perform irrigation, blood collection, dressing changes, and administer medications/fluids through various access points including peripheral, PICC, midline, central venous access, and IVADs. May also change dressings and sites, and administer medications/fluids via subcutaneous access catheters, as well as remove peripheral, PICC, midline, and central venous catheters as directed by the RN.
  • May have the authority to declare death.
  • Collaborates professionally with physicians, healthcare providers, and interdisciplinary team members, ensuring timely communication of relevant information to appropriate healthcare professionals.
  • Records data gathered from assessments and interventions accurately and promptly, adhering to the nursing standards of the organization.
  • Participates in interdisciplinary team meetings as directed by the supervisor.
  • Assigns suitable personal care duties to Hospice Aides and oversees the completion of these tasks.
  • May assist in basic nursing and personal care activities.
  • Engages in educating patients and caregivers on home care practices as directed by the staff nurse.
  • Assists with audits as needed.
  • Participates in the nursing on-call rotation as required, under the supervision of the RN on call.
  • Adheres to agency nursing standards, ensures timely documentation, and demonstrates proficiency in approved infusion therapy tasks.
  • Capable of lifting, turning, and transferring patients; minimum lifting requirement is 40 pounds.
