Job Summary

Hope Services is seeking a highly motivated and compassionate Community Support Facilitator to join our team. As a Community Support Facilitator, you will play a vital role in supporting individuals with developmental disabilities and mental health needs to participate in community activities and receive services.

Key Responsibilities
  • Provide individualized support and leadership to consumers in community settings, facilitating their participation in activities and services defined by their Person Centered Plan and Individualized Personal Plan.
  • Develop and implement strategies to promote community integration, social skills, and independence among consumers.
  • Act as an advocate on behalf of consumers, ensuring their rights and needs are respected and met.
  • Complete required documentation, including incident reports, case recordings, and Individualized Personal Plans.
  • Maintain a professional relationship with consumers, family members, and other stakeholders, representing Hope Services in a positive and respectful manner.
  • Implement behavioral intervention plans to ensure consumer safety and promote learning.
  • Coordinate consumer programming with Center activities, services, and staff.
  • Perform other duties as assigned to ensure the efficiency and effectiveness of program services.
Requirements
  • High school diploma or equivalent, plus two years of related experience or an approved combination of experience and education.
  • Possession of a valid California driver's license and clear driving record.
  • Excellent communication and documentation skills, including proficiency in computerized word processing and database management.
  • CPR and First Aid certification (Hope Services can provide necessary training, if needed).
  • Positive and cooperative attitude toward a consumer-driven service system, working with developmentally disabled adults.
  • Ability to work independently and cooperatively as a member of the team.
Preferred Qualifications
  • Knowledge and understanding of alternative communication systems and assistive technology.
  • Experience working with diverse populations and in community-based settings.
  • Strong interpersonal skills and ability to work effectively with consumers, family members, and other stakeholders.
Work Environment

The Community Support Facilitator will work in a community-based setting, traveling extensively throughout the local community to support consumers. The position requires the ability to work independently and as part of a team, with a focus on providing high-quality support and services to consumers.
