Job Title: Support Coordinator

The Support Coordinator plays a vital role in providing direct support to residents experiencing housing crises and homelessness. This position involves offering clinical engagement and intensive case management services to assist residents in achieving housing stability and connecting them to essential resources.

Key Responsibilities:
  • Provide psychoeducation, supportive counseling, and crisis intervention to help residents achieve housing readiness and stability.
  • Develop and implement tailored Housing Stability Plans, including participant budgets, to support long-term housing success.
  • Coordinate with internal teams, community partners, and external agencies to facilitate optimal outcomes for residents.
  • Maintain accurate client records, ensuring compliance with HIPAA, HMIS databases, HCD, and TCAC standards.
  • Link residents to community resources, such as benefits advocacy, employment services, and mental health support, to support their housing goals.
  • Conduct assessments and provide immediate support during crises, including mandated reporting and safety evaluations.
  • Meet with residents weekly to review progress, re-evaluate case plans, and ensure adherence to tenant responsibilities.
  • Assist clients in applying for permanent subsidies or low-income housing programs, such as Section 8, EHAP, and CalWorks.
Qualifications:
  • Bachelor's degree in social work or a related field, plus 2 years of social services experience, or a Master's degree with 1 year of case management experience.
  • Strong knowledge of social services, public assistance, mental health, and substance abuse systems.
  • Ability to work effectively with diverse populations from a trauma-informed, strength-based approach.
  • Proficient in case management, crisis intervention, and resource coordination.
  • Strong communication, organization, and advocacy skills.
  • Proficient in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint). Familiarity with CES, HMIS/ICMS is a plus.
  • Language: Bilingual in Spanish, required.
  • Licensing: Valid California driver's license and proof of automobile liability insurance.

Pay Details:

$48,000.00 to $58,000.00 per year.
