Job Summary:

UnitedHealth Group is seeking a skilled Field Health Social Services Coordinator/IDD to join our team. As a key member of our healthcare team, you will play a vital role in assessing, planning, and implementing care strategies for patients with Intellectual and Developmental Disabilities (IDD).

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Identify and initiate referrals for social service programs
  • Communicate with stakeholders to ensure quality coordinated care
  • Advocate for patients and families as needed

Requirements:

  • RN in the state of Texas or equivalent
  • 2+ years of experience working with IDD patients
  • Intermediate level of proficiency in MS Word, Excel, and Microsoft Outlook
  • Reliable transportation and ability to travel up to 75% of the time

Preferred Qualifications:

  • 1+ years of experience with long-term care services and support
  • Ability to create, edit, and send documents, spreadsheets, and emails
