Job Summary

The Hospice Social Worker will provide emotional and medical support to terminally ill patients and their families, working closely with the healthcare team to develop and implement a personalized plan of care.

Key Responsibilities
  • Assess patients' and families' psychological responses to terminal illness and provide supportive counseling and teaching.
  • Coordinate with the healthcare team to develop and implement a plan of care that may include at-home treatment and/or admission to an inpatient hospice facility.
  • Explore and evaluate financial situations and assist patients and families in accessing benefits through Medicare, Medicaid, and private insurance.
  • Consult with patients and families regarding funeral arrangements.
  • Make recommendations to the executive director regarding policies and procedures for social work services and bereavement care.
Requirements
  • Master's degree in social work required.
  • License from the state Board of Social Work Examiners.
  • At least 1 year of post-master's degree experience.
  • Valid state driver's license.

Physical Requirements:

  • Good mobility to make visits to a variety of home environments.
  • Good hearing and speech ability to communicate with patients and family.
  • Good vision for driving and assessing patient's physical condition.
