Healthcare Social Services Coordinator Role

Key Responsibilities:
- Assess and implement care strategies tailored to individual patients' needs.
- Identify and initiate referrals for social services, including financial, psychosocial, community, and state supportive services.
- Manage care plans throughout the continuum of care as a single point of contact.
- Communicate with stakeholders to ensure quality coordinated care and services are provided efficiently.
- Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.

Requirements:
- RN in the state of Texas or equivalent licensure.
- 2+ years of experience working directly with people with Intellectual and Developmental Disability (IDD).
- Intermediate level of proficiency in MS Word, Excel, and Microsoft Outlook.
- Reliable transportation and ability to travel up to 75% of the time.
Preferred Qualifications:
- 1+ years of experience with long-term care services and support, Medicaid, or Medicare.
- Ability to create, edit, save, and send documents, spreadsheets, and emails.
