As a key member of our team, you'll assess, plan, and implement care strategies that cater to individual patient needs. You'll also identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.

With a focus on quality coordinated care and services, you'll communicate with all stakeholders to ensure that patients receive the support they need. Your advocacy skills will be essential in representing patients and families, ensuring their needs and choices are fully represented and supported by the healthcare team.

As a Field Social Services Coordinator, you'll have the flexibility to work remotely and travel up to 75% of the time to visit Medicaid members in their homes and other settings. You'll need reliable transportation and access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
