Licensed Medical Social Worker

Responsibilities:
- Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
- Communicate findings to the RN Case Manager and other members of staff, including the interdisciplinary group.
- Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources.
- Supervise the implementation of an individualized social work plan of care and notify the Bereavement Coordinator of bereavement assessment needs.
- Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings.
- Provide consultation, education, and support to the interdisciplinary team on social work needs.
- Follow policies and procedures consistent with recognized standards of professional practice.
- Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores.
- Collaborate with your Clinical Director and entire team to provide a "concierge" level hospice experience.
Qualifications: The ideal candidate will have a Master's degree in Social Work from an accredited school of social work and a current license of Social Work in Texas. A minimum of one year of experience as a social worker in long-term care or medical surgical/acute care setting is required. One year of hospice experience is preferred but not required. The candidate should demonstrate mastery of all facets of hospice care and display a communication style that makes everyone who works with them want to do their best. They should be happy in the field providing patient care and embrace new opportunities to learn and grow.
