End-of-Life Care Specialist

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Eden Health. As an End-of-Life Care Specialist, you will provide in-home services to patients in Clark County, working closely with their families and caregivers to ensure the highest quality of life.
About the RoleThis is a rewarding opportunity for a motivated and empathetic RN to make a meaningful difference in the lives of our patients and their loved ones. You will be responsible for managing patient care plans, collaborating with interdisciplinary teams, and ensuring seamless transitions between healthcare settings.
Responsibilities- Develop and implement comprehensive patient care plans that address physical, emotional, and spiritual needs
- Collaborate with physicians, social workers, and other healthcare professionals to ensure coordinated care
- Conduct thorough assessments of patient conditions, identifying areas for improvement and implementing interventions as needed
- Provide education and support to patients and families on end-of-life care, pain management, and symptom control
- Manage caseloads effectively, prioritizing patient needs and communicating regularly with multidisciplinary teams
- Maintain accurate and up-to-date records of patient interactions, treatments, and outcomes
To succeed in this role, you will need:
- A valid RN license in the state of practice
- 1+ year of experience in hospice care, home health, or acute-care settings
- Strong assessment and critical thinking skills, with ability to analyze complex patient situations
- Excellent communication and interpersonal skills, with capacity to build rapport with patients, families, and healthcare teams
- Ability to work independently, prioritize tasks, and manage competing demands
- Familiarity with electronic medical records (EMRs) and proficiency in Microsoft Office Suite
