Position Overview:
We are seeking a detail-oriented and dedicated professional to join our team as a Patient Care Coordinator. This role is instrumental in providing a seamless patient experience and ensuring all documentation and information is collected and organized appropriately. Successful candidates will have a passion for providing a high level of customer service to home care patients.
Patient Care Coordinator Responsibilities:
- Send and track required face-to-face documentation for Physicians to sign and return
- Collect information for currently hospitalized clients: H&P (History & Physical), ROC (Resumption of Care), MD orders, etc.
- Conduct pre-admit calls to verify client information, as well as five-day and discharge follow-up calls
- Track hospitalized clients and notify Community Liaisons of discharges, in order to resume providing care
Patient Care Coordinator Qualifications:
- High school graduate; college degree preferred
- Knowledge of medical terminology is strongly preferred
- Minimum of one (1) year of experience in an office or health care setting preferred
- Strong verbal, written, and telephonic communication skills
- Demonstrated ability to type at least 50 words per minute
