Position Overview

Schedule: Part Time, 20 hours per week with weekend and on-call rotation.

Location: Bedford, PA

Role Summary:
Under the guidance of the Home Care Supervisor and in collaboration with the Interdisciplinary Team (IDT), the Home Care Coordinator, who is a Certified Occupational Therapy Assistant (COTA/L), plays a crucial role in assessing and coordinating the provision of home and personal care services for adults aged 55 and older. This position requires a deep understanding of the needs of frail elderly individuals and the ability to plan and deliver care in accordance with established standards in gerontological nursing and Community LIFE protocols. Mileage expenses will be reimbursed for home visits.

Educational Requirements: Graduate from an accredited Certified Occupational Therapist Assistant (COTA) program.

Experience Requirements: A minimum of one year of experience working with a frail elderly demographic, ideally in long-term care, home care, or community health settings. Experience in educating patients and collaborating with healthcare professionals is preferred.

Licensure and Certifications: Valid COTA license in Pennsylvania and a current Pennsylvania Driver's license.

  • Strong written and verbal communication abilities.
  • Excellent organizational and planning skills, with the capacity to manage multiple tasks effectively.
  • Ability to work autonomously and apply critical thinking in decision-making.
  • Familiarity with utilization review, quality assurance, and managed care principles.
  • Capability to collaborate within an interdisciplinary team to enhance elderly care.
  • Understanding of local healthcare and geriatric service networks.
  • Basic proficiency in Windows operating systems, email, and word processing applications.
  • Commitment to delivering services in a compassionate, responsive, and courteous manner; dependable, resourceful, and adaptable.
  • Ability to engage effectively with staff, participants, providers, and referral sources.
  • Genuine interest in geriatrics and community-based programming.
  • Appreciation for and enjoyment in working with elderly individuals.
