Role Summary:
Under the guidance of the Home Care Supervisor and in collaboration with the Interdisciplinary Team (IDT), the Home Care Coordinator, a Certified Occupational Therapy Assistant (COTA/L), is responsible for utilizing technical, organizational, and communication skills to effectively evaluate the needs and coordinate the provision of home and personal care services for adults aged 55 and older. The Home Care Coordinator must exhibit the necessary knowledge and skills to plan and deliver care to frail elderly individuals, adhering to established responsibilities and standards of practice in gerontological nursing as well as Community LIFE standards. Mileage reimbursement is provided for home visits.

Educational Requirements: Graduate of an accredited Certified Occupational Therapist Assistant (COTA) program.

Experience Requirements: A minimum of one year of experience working with a frail elderly population, ideally in long-term care, home care, or community health settings. Experience in educating patients and other healthcare professionals is preferred.

Licensure and Certification: Valid Pennsylvania COTA license and a valid Pennsylvania driver's license.

Essential Skills:

  • Understanding of the medical, social, and emotional needs of a frail elderly population.
  • Strong written and verbal communication abilities.
  • Excellent organizational and planning skills; capable of managing multiple priorities.
  • Ability to work independently and apply critical decision-making skills.
  • Familiarity with utilization review, quality assurance, and managed healthcare concepts.
  • Capability to collaborate with an interdisciplinary team to provide care for the elderly.
  • Knowledge of local healthcare and geriatric service networks.
  • Basic proficiency in Windows operating systems, email, and word processing.
  • Ability to deliver services in a compassionate, responsive, and courteous manner; dependable, resourceful, and adaptable.
  • Proficient in working effectively with staff, participants, providers, and referral sources.
  • Interest in geriatrics and community-based programming.
  • Appreciation for and enjoyment in working with elderly individuals.
