Role Summary:
Under the guidance of the Home Care Supervisor and in collaboration with the Interdisciplinary Team (IDT), the Geriatric Care Coordinator, a Certified Occupational Therapy Assistant (COTA/L), is responsible for applying technical, organizational, and communication skills to effectively evaluate the requirements for and coordinate the provision of home and personal care services for adults aged 55 and older. The Coordinator must exhibit the necessary knowledge and skills to plan care and deliver services to frail elderly individuals in accordance with established responsibilities and standards of practice in gerontological nursing as well as Community LIFE standards. Mileage expenses will be reimbursed for home visits.

Educational Requirements: Graduate of an accredited Certified Occupational Therapist Assistant (COTA) program.

Experience Requirements: A minimum of one year of experience working with a frail elderly demographic, ideally in long-term care, home care, or community health settings. Experience in educating patients and other healthcare professionals is preferred.

Licensure and Certifications: Valid Pennsylvania COTA license and a valid Pennsylvania Driver's license.

Essential Skills:

  • Comprehensive understanding of the medical, social, and emotional needs of a frail elderly population.
  • Proficient written and verbal communication abilities.
  • Strong organizational and planning capabilities; adept at managing multiple priorities.
  • Ability to work autonomously and employ critical decision-making skills.
  • Familiarity with utilization review, quality assurance, and managed healthcare concepts.
  • Capability to collaborate effectively within an interdisciplinary team to provide care for the elderly.
  • Knowledge of local healthcare and geriatric service networks.
  • Basic proficiency in Windows operating systems, email, and word processing applications.
  • Ability to deliver services in a compassionate, responsive, and courteous manner. Dependable, resourceful, and adaptable.
  • Skill in working effectively with staff, participants, providers, and referral sources.
  • Genuine interest in geriatrics and community-based programming.
  • Appreciation for and enjoyment in working with elderly individuals.
