Role Summary:
As a Geriatric Care Coordinator, you will operate under the guidance of the Home Care Supervisor and collaborate closely with the Interdisciplinary Team (IDT). This role involves utilizing your technical, organizational, and communication expertise to effectively evaluate the requirements for, and orchestrate the provision of, home and personal care services tailored for adults aged 55 and older. The Geriatric Care Coordinator is expected to exhibit the necessary knowledge and skills to devise care plans and deliver services to elderly individuals in accordance with established responsibilities and standards of practice in gerontological care.

Essential Qualifications:

  • Graduation from an accredited Certified Occupational Therapist Assistant (COTA) program.

Experience Requirements:

  • A minimum of one year of experience working with a frail elderly demographic, ideally in long-term care, home care, or community health settings.
  • Experience in educating patients and collaborating with other healthcare professionals is preferred.

Licensure and Certifications:

  • Possession of a valid PA COTA license.
  • Valid PA Driver's license.

Key Competencies:

  • Understanding of the medical, social, and emotional requirements of a frail elderly population.
  • Proficient written and verbal communication abilities.
  • Strong organizational and planning capabilities; adept at managing multiple tasks.
  • Ability to work autonomously and apply critical thinking skills.
  • Familiarity with utilization review, quality assurance, and managed care principles.
  • Capacity to collaborate effectively within an interdisciplinary team focused on elderly care.
  • Knowledge of local healthcare and geriatric service networks.
  • Basic proficiency in Windows operating systems, email, and word processing applications.
  • Ability to provide services in a compassionate, responsive, and courteous manner; dependable, resourceful, and adaptable.
  • Skill in working effectively with staff, participants, providers, and referral sources.
  • Genuine interest in geriatrics and community-based programming.
  • Appreciation for and enjoyment in working with elderly individuals.
