Family Support Specialist

The Family Support Specialist plays a crucial role in fostering connections, providing assistance, and empowering families with youth who are exhibiting at-risk behaviors within the community. This position requires effective communication with all family members while honoring and valuing their cultural backgrounds and beliefs.

Key Responsibilities:

  • Evaluate the strengths and needs of children, youth, and families.
  • Support families in achieving their Plan of Care (POC) objectives.
  • Encourage families to take ownership of their care plans.
  • Incorporate family values and cultural aspects into personalized service coordination planning.
  • Manage a diverse caseload of up to twenty families, ensuring that meeting frequency aligns with planning needs.
  • Work independently and meet families at their convenience, which may include early mornings, evenings, or weekends.
  • Establish and maintain strong partnerships with service agencies and providers.
  • Guide families in developing a comprehensive plan to identify problems, provide emotional support, assess needs and strengths, and locate resources, including services, entitlements, and community supports.
  • Assist families and youth in creating their own community-based support networks, ensuring their goals are integrated into a cohesive plan.
  • Collaborate with Care Coordinators throughout the Wraparound process, including intake, family engagement, team development, and task implementation within the POC.
  • Ensure families are active participants in planning, executing, monitoring, and evaluating their care plans.

Qualifications:

  • High school diploma or equivalent.
  • Personal experience as a primary caregiver for a youth with significant behavioral or mental health challenges is essential. Preference will be given to those who can articulate their experiences and navigate available services.
  • Previous experience in assisting families to access appropriate services and supports is preferred.
  • Familiarity with local and state resources, particularly those serving early childhood populations.
  • Strong active listening skills to provide non-judgmental peer support and engagement.
  • Excellent written and verbal communication abilities.
  • Valid Maryland driver's license and access to a personal vehicle; travel within the state is required.
