We are seeking a skilled and compassionate Admissions Nurse to join our team at Res-Care, Inc. As an Admissions Nurse, you will play a vital role in supporting our patients by providing direct care and coordinating with our interdisciplinary team to ensure that both patients and their families have an optimal care experience.

Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with our healthcare team to ensure seamless care coordination and effective communication.
  • Administer medication and treatment as prescribed by the physician in the plan of care.
  • Counsel patients and families on meeting their nursing and related needs.
  • Document patient progress and maintain accurate clinical notes.
  • Participate in interdisciplinary group meetings to discuss patient care and address any concerns.
  • Ensure compliance with all state and federal legal and regulatory requirements.
Requirements
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in home health or a similar setting is highly desirable.
  • Commitment to clinical and documentation excellence.
