Job Summary

This is an exciting opportunity for a skilled and compassionate Registered Nurse (RN) to join our team as a Behavioral Health RN Case Manager. As a key member of our interdisciplinary team, you will be responsible for managing a caseload of behavioral health patients, assessing their health, psychosocial needs, and ensuring they receive the care they deserve.

Responsibilities:

  • Manage a caseload of behavioral health patients
  • Assess patient health and psychosocial needs
  • Craft personalized care plans with an interdisciplinary team
  • Communicate and collaborate with physicians, clinical supervisors, and healthcare team members
  • Empower patients, families, and facility staff through education and support
  • Supervise and mentor behavioral health aides and LPNs
Requirements

To be considered for this rewarding opportunity, you must possess the following qualifications:

  • Current RN License (unrestricted in the state)
  • 1+ year of clinical experience (Behavioral Health, Home Health, or Hospice experience preferred)
  • Reliable Transportation: Must have a dependable vehicle, valid driver's license, and current auto insurance to visit patients
  • 50% Travel Required within the service region
