Case Manager Behavioral Health RN Specialist

The ideal candidate will be a highly skilled and compassionate RN with 1+ year of clinical experience in Behavioral Health, Home Health, or Hospice. This is a full-time salaried position located in Webster, Monday-Friday, 8:00 AM - 5:00 PM.
About the RoleThis is an exciting opportunity to join our elite team of healthcare professionals providing the Right Care, at the Right Time, in the Right Place. As a Behavioral Health RN Case Manager, you will play a vital role in managing a caseload of behavioral health patients, assessing their health, psychosocial needs, and ensuring they receive the care they deserve.
Key Responsibilities:
- Develop and implement personalized care plans with an interdisciplinary team
- Collaborate with physicians, clinical supervisors, and healthcare team members to ensure top-quality care and timely interventions
- Provide education and support to patients, families, and facility staff
- Supervise and mentor behavioral health aides and LPNs
