The Role
This is a full-time salaried position, based in Webster, with an estimated salary of $80,000 per year. As a Behavioral Health RN Case Manager, you will be responsible for managing a caseload of behavioral health patients, assessing their needs, and developing personalized care plans.
Key Responsibilities:
- Manage a caseload of behavioral health patients, assessing their health, psychosocial needs, and ensuring they receive the care they deserve.
- Develop and implement personalized care plans in collaboration with interdisciplinary teams.
- Partner with physicians, clinical supervisors, and healthcare team members to ensure top-quality care and timely interventions.
- Empower patients, families, and facility staff through education and support, enhancing the care experience.
- Supervise and mentor behavioral health aides and LPNs, ensuring consistent, high-quality care delivery.
Requirements:
- Current RN License (unrestricted in the state)
- 1+ year of clinical experience (Behavioral Health, Home Health, or Hospice experience preferred)
- Reliable transportation, valid driver's license, and current auto insurance
- 50% Travel Required within the service region
- Educational Background: Associate's or Bachelor's Degree in Nursing (preferred)
