Job Overview

Position Summary

The Case Manager at Hillcrest Children Center collaborates closely with the Certified Addictions Counselor II, Clinical Manager, and Quality Analyst/Care Coordinator to deliver comprehensive case management services to individuals grappling with substance use or related disorders. This role is pivotal in assessing client needs, crafting, executing, and evaluating treatment plans, as well as coordinating essential services for clients and their families.

Key Responsibilities

  • Accepts newly assigned cases following the completion of diagnostic intake by a licensed professional.
  • Conducts thorough case management assessments for all clients entering the caseload.
  • Facilitates prompt connections to Psychiatrist, Nurse Practitioner, or Registered Nurse to ensure access to primary care, mental health services, substance abuse counseling, medication management, community support, residential treatment, and employment assistance.
  • Develops tailored treatment/service plans in collaboration with clients to implement necessary services and foster effective partnerships.
  • Provides ongoing support and case management functions aligned with the identified problems, needs, and strategies within the treatment plan to assist clients in achieving their goals.
  • Conducts regular face-to-face reviews with clients and relevant staff to monitor progress towards treatment goals, allowing for necessary adjustments to the treatment plan.
  • Maintains timely documentation of all interactions, including interviews and meetings with treatment providers, ensuring compliance with regulatory standards.
  • Assists clients in securing appointments with external providers and navigating community services.
  • Refers clients to community resources as required.
  • Establishes and nurtures relationships with identified service providers, ensuring prompt completion of referrals.
  • Maintains accurate tracking and documentation of referral processes.
  • Communicates effectively with the treatment team regarding any issues or needs for follow-up.
  • Engages with clients and/or family members about pending referrals and diagnostic assessments.
  • Sends necessary notifications and reminders to clients regarding referral statuses.
  • Reviews referral details and expectations with clients to ensure clarity.
  • Ensures the electronic health record is current with information on specialist consultations, including scheduled appointments, results, and treatment recommendations.
  • Meets regularly with the immediate supervisor to enhance professional development, review case management activities, and address administrative matters.
  • Maintains close communication with the treatment team and support staff regarding medication adherence, side effects, and any changes that may require intervention.
  • Participates in regular interdisciplinary staff meetings at the position location.
  • Performs other related projects and duties as assigned.

Educational Qualifications

  • Bachelor's degree is required; a Master's degree is preferred.
