LPN - Patient Care Specialist

At CommonSpirit Health Mountain Region, we are committed to delivering high-quality patient care and promoting a culture of compassion and excellence. As an LPN - Patient Care Specialist, you will play a vital role in providing individualized care to our patients, working collaboratively with the healthcare team to identify and meet their physical, emotional, and spiritual needs.

Key Responsibilities:
  • Perform patient observation and data collection related to patient care
  • Administer medication, treatment, and procedures according to physician orders and resident care plans
  • Communicate patient findings to supervisors and facilitate collaboration with the healthcare team
  • Oversee care provided by CNAs, Care Partners, QMAPs, and other support staff
Requirements:
  • One year of previous LPN experience preferred
  • Knowledge in nursing and person-centered care concepts
  • Ability to demonstrate competency in individual care of patients based on assessed age and developmental needs
  • HS GED
  • Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only)
  • BLS
