Position Overview


As a Registered Nurse Care Coordinator, you will play a vital role in overseeing and delivering direct nursing services to participants within the PACE (Program of All-Inclusive Care for the Elderly) framework across diverse environments, including clinics, care centers, home settings, virtual consultations, skilled nursing facilities, and more.


This position is integral to the Interdisciplinary Team (IDT), collaborating closely to conduct assessments and advance participant care strategies.

Key Responsibilities

  • Act as a key member of the Interdisciplinary Team (IDT), contributing to thorough patient evaluations, detailed care plans, delegation of tasks, and prompt service determination requests.
  • In collaboration with a medical provider, spearhead personalized care for a designated group of participants, serving as the primary contact for the IDT to ensure adherence to each participant's care plan.
  • Provide care alongside team members in various locations where participants reside (home, care center, virtual, assisted living facilities, skilled nursing facilities), addressing needs such as basic primary care, medication management, wound treatment, oxygen therapy, and other clinical requirements in line with your professional scope.
  • Conduct comprehensive nursing assessments that encompass physical, psychosocial, and behavioral evaluations.
  • Recognize emergency situations and independently initiate appropriate interventions within your scope of practice.
  • Educate participants, caregivers, and staff on personalizing and implementing care plans, as well as responding effectively to emerging situations in participants' lives.
  • Monitor and assess therapeutic interventions, recommending adjustments as necessary to proactively enhance health and well-being.
Schedule and Shift Details

40 hours per week, Monday through Friday, with occasional on-call responsibilities. Schedules may vary.
Qualifications

Required:
  • Associate degree or higher.
  • 3+ years of experience in Wound Care.
  • 4+ years as a Registered Nurse.
  • 3+ years of experience in any of the following areas: Geriatric Care, Senior Care, Geriatric/Long-Term Care.
  • Valid American Heart Association Basic Life Support (AHA BLS) certification.
  • Valid Registered Nurse (RN) license in the state of California.
Preferred:
  • Valid Alzheimer's certification.
