Nurse Practitioner

Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team at South Peninsula Hospital. As a Registered Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities:
- Provide skilled nursing assessments and direct care to patients in our Long Term Care facility
- Develop and implement individualized care plans to meet the unique needs of each patient
- Administer medications and treatments as prescribed by physicians
- Monitor and coordinate care performed by assigned staff to ensure quality and safety
- Participate in the development and implementation of policies and procedures to improve patient care and outcomes
Requirements:
- Minimum of one year experience in Gerontology, preferably in a Long Term Care facility
- Strong communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
- Basic computer skills and ability to complete electronic documentation
