The Community You Will Serve:
+ Our flagship facility, AdventHealth Orlando, is a 1,368-bed hospital situated on a beautiful tropical campus, serving as a key tertiary center for the Southeast, the Caribbean, and South America.
+ The hospital features one of the largest Emergency Departments and cardiac catheterization labs in the nation, catering to over 32,000 inpatients and 125,000 outpatients annually.
Your Role:
*The Care Management Supervisor operates under the guidance of the Care Management Manager or Facility Director, assisting in
overseeing daily operations and supervising team members within the Care Management department.* This role acts as the primary liaison for Care Managers, Social Workers, and patients, addressing any barriers or concerns that may arise in inpatient settings.
Key Responsibilities:

* Supervise daily discharge planning and social services operations in both inpatient and emergency department settings as directed by the Care Management Manager or Facility Director.
* Serve as the main point of contact for any Care Management-related issues on inpatient units.
* Manage personal assignments and outcomes for patients and departments under direct supervision.
* Collaborate with the Care Management Manager/Facility Director to implement corporate strategies.
* Ensure active participation of staff in daily Multidisciplinary Rounds.
* Act as a resource for stakeholders regarding medical necessity, patient status, and discharge criteria.
* Ensure staff documentation adheres to guidelines through appropriate monitoring and coaching.
* Facilitate care conferences for complex transitions and placements as necessary.
* Participate in hospital and medical staff meetings to discuss patients meeting criteria for extended stays or difficult placements.
* Report quality and risk issues to relevant departments in a timely manner.
* Address complaints from patients, staff, and physicians with the Care Management Manager/Facility Director, providing written reports as needed.
* Demonstrate a proactive problem-solving approach to staff, physicians, and peers.
* Act as a supportive change agent for Care Management processes.
* Encourage team engagement and maintain open communication with staff.
* Assist with performance evaluations and support corrective action plans.
* Balance schedules to meet departmental needs prior to posting.
* Communicate any schedule changes to minimize unscheduled absences.
* Plan job responsibilities for optimal staff utilization and resolve daily staffing challenges.
Minimum Qualifications:
Registered Nurse:
Associate's Degree in Nursing OR
Social Worker:
BSW with two (2) years of direct Care Management experience. State-specific RN license required for nurses.
Preferred Qualifications:
Registered Nurse:
BSN
Social Worker:
MSW with three (3) years of direct Care Management experience, and certifications such as CCM, ACM, or RN-BC in Care Management.
