Position Overview:
The Care Management Manager operates under the general guidance of the Care Management Facility Director, overseeing daily departmental operations and supervising team members.
Core Responsibilities:
- Coordination of Care
- Ensuring compliance with Centers for Medicare & Medicaid Services (CMS) Conditions of Participation
- Implementing Social Services processes based on data management
- Managing accountability for Length of Stay, Excess Days, Avoidable Days, Readmission Prevention, and Discharge Planning
- Enhancing department engagement, customer experience, and compliance with regulatory standards, including conducting employee evaluations
- Fostering professional and collaborative relationships with the Interdisciplinary Team
Qualifications Required:
Minimum Qualifications:
- Registered Nurse: BSN or equivalent
- Social Worker: BSW or a Master's degree in a related field
- A minimum of 3 years of direct experience in Care Management/Utilization Management within an acute care environment
Preferred Qualifications:
- Registered Nurse: BSN or a Master's degree in a related field
- Social Worker: MSW or LCSW
- At least 2 years of supervisory or managerial experience
- RN-BC Certification in Care Management
