Personal Care Nurse Specialist

Aveanna Healthcare is seeking a skilled and compassionate Personal Care Nurse Specialist to join our team. As a Registered Nurse (RN), you will provide high-quality nursing care in the comfort of patients' homes, promoting independence and dignity.

Responsibilities- Deliver comprehensive nursing care, including medication administration, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, incorporating patient goals and preferences.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and communication.
- Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
- Conduct regular assessments and evaluations to adjust care plans as needed.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current, unrestricted state license as a Registered Nurse.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
