About the Role

Aveanna Healthcare seeks a dedicated and compassionate Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. As a Skilled RN Care Specialist, you will play a vital role in delivering personalized patient care while adhering to strict confidentiality standards and professional boundaries.

Key Responsibilities
  • Patient Care Coordination: Deliver and document skilled nursing care according to developed care plans and physician orders, ensuring seamless coordination of quality patient care.
  • Ongoing Assessment: Continuously observe and assess patient condition, reporting changes to supervisors and physicians as necessary, to ensure optimal patient outcomes.
  • Care Documentation: Accurately document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner, maintaining detailed records for future reference.
Requirements
  • Nursing Qualifications: Current, unrestricted state license as a Registered Nurse; CPR certification; and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies/procedures.
  • State-Specific Requirements: One year of experience required for specific states, including pediatrics, working under current nursing license, or experience as a licensed nurse.
  • Discipline-Specific Requirements: Two years of experience for medical oncology, ambulatory, medical surgical, and peri-operative care RNs.
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting; home health experience.
