Job Description:

We are seeking a dedicated and compassionate Registered Nurse to fill the role of RN Healthcare Coordinator. In this position, you will play a critical role in coordinating care and services for patients with complex medical needs.

About the Job:

  1. Assess patient needs, develop individualized care plans, and coordinate services to promote optimal health outcomes.
  2. Collaborate with healthcare providers, community organizations, and social services to ensure seamless patient care.
  3. Provide education and support to patients and their families on disease management, self-care, and healthy living.
  4. Manage patient panels, track patient needs, and proactively coordinate care to optimize health outcomes.
  5. Participate in multidisciplinary teams to develop and implement care plans.

Requirements:

  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN).
  • Minimum 2 years of experience in case management, healthcare coordination, or related field.
  • Current RN licensure in New Mexico or compact state.
  • Strong communication and interpersonal skills.
  • Bilingual English/Spanish proficiency preferred.
