We are seeking a skilled and compassionate Registered Nurse to join our team as a Care Coordinator RN. This is a full-time position offering a competitive salary of $34-$42 per hour, based on location in Albuquerque.

Job Summary

The Care Coordinator RN will be responsible for providing comprehensive care to patients with diverse and complex health conditions. This includes conducting nursing assessments, administering medications, and collaborating with healthcare teams to develop effective care plans.

  • Perform thorough nursing assessments to identify patient needs and develop individualized care plans.
  • Administer medications and treatments as ordered by physicians, ensuring accurate dosing and monitoring for potential side effects.
  • Collaborate with interdisciplinary teams to coordinate patient care, including referrals to specialists and community resources.
  • Delegate tasks to staff members, supervising their work to ensure quality care delivery.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
Required Skills and Qualifications

To be successful in this role, the ideal candidate will possess:

  • A current license to practice as a Registered Nurse in the State of New Mexico.
  • CPR certification.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.
  • Excellent critical thinking and problem-solving skills, with the ability to analyze complex situations and develop effective solutions.
  • Ability to work independently and as part of a team, prioritizing tasks and managing time effectively in a fast-paced environment.
