Healthcare Coordinator Specialist

We are seeking an experienced Healthcare Coordinator Specialist to join our team as an Oncology Patient Navigator. This role is a critical part of our patient care process, and we need someone who can effectively coordinate patient care and provide exceptional patient experiences.
About the RoleAs a Healthcare Coordinator Specialist - Oncology Patient Navigation, you will be responsible for performing visit closure activities, coordinating patient care, and providing lay navigation to oncology patients. You will work closely with interdisciplinary care plan teams and nurse navigation teams to assess oncology patient needs and coordinate plans of care.
This role requires strong written communication skills, ability to communicate effectively with members, employer groups, and physicians, and being customer-focused. You must have 2+ years of healthcare experience, a High School Diploma, and BLS certification. A Lay Navigator certificate is required within 6 months of hire.
Responsibilities- Prepare and gather medical records
- Participate in interdisciplinary care plan teams
- Assist nurse navigation teams with assessment of oncology patient needs and coordination of plan of care
- Provide lay navigation to oncology patients
- Identify clinical patient care needs and direct them to the Navigator
- Advanced scheduling
- Recognizing payor requirements
- Processing daily referral workqueues
- Strong written communications
- Ability to communicate effectively with members, employer groups, and physicians
- Customer-focused
