About the Role

We are seeking a highly skilled Nurse Care Coordinator to join our team at Professional Case Management. As a key member of our home healthcare team, you will be responsible for coordinating and overseeing nursing care for clients in their homes, ensuring compliance with agency policies and state regulations.

Key Responsibilities
  • Case management: Coordinate and oversee nursing care for clients in their homes, ensuring compliance with agency policies and state regulations.
  • Staff supervision: Supervise and manage a team of RN case managers, providing guidance and support as needed.
  • Collaboration: Work closely with regional directors to improve service delivery and ensure alignment with agency goals.
Requirements
  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years of nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
Compensation and Benefits

The salary for this role is estimated at $42 per hour, plus shift differentials where applicable. Additionally, we offer a range of benefits, including bonus eligibility and opportunities for flexible scheduling.
