Healthcare Access Coordinator Position

We are recruiting for a Healthcare Access Coordinator to support our mission of providing high-quality primary medical, dental, and behavioral healthcare to underserved populations. As a key member of our team, you will facilitate patient access to community support programs, educational initiatives, and healthcare resources. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse stakeholders, including patients, providers, and external partners.
Key Responsibilities:
The successful candidate will coordinate patient care, conduct chart reviews, participate in quality improvement activities, and communicate with patients and staff regarding care plans and outcomes. You will also assist with developing and implementing healthcare quality improvement initiatives and population health activities. A strong working knowledge of medical terminology and office equipment operation is essential for this role.
Qualifications:
To succeed in this position, you must have a completion of medical assistant training or comparable experience, direct work experience in a healthcare setting, and knowledge of medical terminology. A minimum certified/registered Medical Assistant certification and CPR certification are also required. Bi-lingual English/Spanish language proficiency is highly preferred.
