Housing Case Manager

  2. Develop Individualized Housing Plans: Collaborate with clients to create personalized housing plans focusing on permanent housing solutions.
  3. Provide Ongoing Case Management: Offer regular check-ins, goal setting, and progress monitoring to support clients in achieving their housing goals.
  4. Access Supportive Services: Assist clients in accessing supportive services such as mental health counseling, substance abuse treatment, and job training.
  5. Advocate for Clients' Rights: Advocate for clients' rights and access to housing resources within the community.
  6. Collaborate with Community Partners: Work with landlords, property managers, and housing agencies to secure affordable and safe housing options for clients.
  7. Maintain Accurate Records: Maintain up-to-date client records, progress notes, and case documentation.
  8. Participate in Team Meetings: Participate in multidisciplinary team meetings and case conferences to coordinate services and support for clients.
  9. Stay Informed about Housing Policies: Stay informed about relevant housing policies, programs, and resources to better serve clients and advocate for systemic change.
  10. Provide Crisis Intervention: Provide crisis intervention and support during housing transitions or emergencies.
Qualifications:
  1. Bachelor's Degree: Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).
  2. Experience Working with Homeless Populations: Previous experience working with homeless populations or in a housing-focused case management role.
  3. Strong Understanding of Housing Issues: Strong understanding of housing issues, homelessness prevention strategies, and housing resources in the local community.
  4. Excellent Communication Skills: Excellent communication skills, both verbal and written, with the ability to collaborate effectively with clients, colleagues, and community partners.
  5. Cultural Competence: Empathy, patience, and cultural competence in working with diverse populations, including individuals with mental health or substance use challenges.
  6. Ability to Work Independently: Ability to work independently, manage a caseload, and prioritize tasks in a fast-paced environment.
  7. Proficiency in Microsoft Office Suite: Proficiency in Microsoft Office Suite and case management software.
