Job Summary

We are seeking a highly skilled and detail-oriented Bilingual Referral Coordinator to join our team at Triad Adult and Pediatric Medicine, Inc. As a key member of our patient care team, you will be responsible for coordinating referrals to outside medical facilities, clinics, and agencies, ensuring seamless patient care and promoting community awareness through health education and marketing initiatives.

Key Responsibilities
  • Referral Coordination: Coordinate patient referrals to outside specialists, physicians, testing facilities, specialized schools, agencies, and parent support/advocacy services, utilizing appropriate procedures.
  • Family Support: Assist and advise families with available service assessments, ensuring they receive the necessary support and resources.
  • Liaison Services: Serve as a liaison for county and member hospital and agencies, fostering strong relationships and facilitating effective communication.
  • Tracking and Data Management: Track TAPM in-patients at hospitals, participate in discharge planning rounds, and maintain accurate records of referrals and lab/studies.
  • Community Outreach: Promote community awareness of TAPM by providing patient education in community forums, such as health fairs, daycare centers, schools, malls, drug stores, and other community/agency functions.
  • Interpretation and Translation: Provide interpreters for non-English speaking and/or hearing impaired patients/families, ensuring language access and cultural sensitivity.
  • Authorization and Reporting: Obtain authorization from other facilities for treating TAPM patients and ensure reports are available for primary providers.
  • Transportation and Logistics: Coordinate transportation needs, including providing maps, distributing bus tickets, and maintaining accurate records.
Requirements and Qualifications

To be successful in this role, you will need:

  • Excellent Communication Skills: Strong verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
  • Cultural Competence: Ability to work with diverse populations, including non-English speaking and/or hearing impaired individuals.
  • Organizational Skills: Strong organizational skills, with the ability to prioritize tasks, manage multiple projects, and meet deadlines.
  • Technical Skills: Proficiency in relevant software and systems, including electronic health records and referral management systems.
