About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Adoration Home Health and Hospice. As a PRN LPN, you will provide high-quality care to our clients in the comfort of their own homes.

Responsibilities
  • Provide 1:1 care for clients, following their individualized plan of care and maintaining their dignity and independence.
  • Monitor clients' vital statistics and report any abnormal findings to the Case Manager and Physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
  • Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
  • Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
  • Ensure all patient needs are being met in accordance with the instructions of the physician.
  • Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
Qualifications
  • LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
