Patient Care Manager

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at Ochsner Home Health of New Orleans. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care in the comfort of their own homes.
Key Responsibilities- Supervise and Coordinate Clinical Services: Oversee the provision of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
- Provide Clinical Services: Deliver clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
- Coordinate Patient Care: Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Lead and Develop Team Members: Provide leadership development opportunities and support to patient-facing teams, promoting a culture of caring, belonging, and trust.
- Current RN Licensure: Hold a current RN license in the state of practice.
- Clinical Experience: Possess at least one year of clinical experience, with no restrictions on RN licensure.
- Certifications and Education: Maintain current CPR certification and have access to a dependable vehicle or public transportation.
