Key Responsibilities- Direct and Initiate Clinical Pharmacy Services: Develop, implement, and maintain clinical pharmacy services to improve patient outcomes and reduce costs.
- Provide Clinical Consultation: Offer expert advice to medical staff and other healthcare providers on therapeutic assessments and disease management.
- Communicate and Document Activities: Effectively communicate and document all activities and interventions with prescribers, pharmacists, and other healthcare professionals.
- Collaborate with Healthcare Providers: Work closely with healthcare providers to ensure timely resolution of concerns and questions.
- Develop Clinical Guidelines and Protocols: Facilitate the development and implementation of treatment guidelines, pathways, and protocols to improve patient care.
- Participate in Clinical Committees: Actively participate in clinical committees, including the pharmacy and therapeutics committee, to stay up-to-date on regulatory standards and best practices.
- Pharm.D. Degree: Possess a Doctor of Pharmacy degree from an accredited institution.
- Experience: Have at least 2 years of experience in inpatient pharmacy hospital/clinical capacity or completion of a Pharmacy Residency.
- Licensure: Be currently licensed or registered to practice pharmacy in Louisiana.
- Board Certification: Possess a Board Certified Pharmacotherapy Specialist (BCPS) certification.
