Job Description

We are seeking a highly skilled Patient Care Manager to join our team at Ochsner Home Health of New Orleans. As a Patient Care Manager, you will play a critical role in the supervision and coordination of clinical services, ensuring that patients receive high-quality care in the comfort of their own homes.


Key Responsibilities:

  • Supervise and coordinate clinical services to ensure that patients receive timely and effective care.
  • Provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations, as appropriate.

Requirements:

  • Current RN licensure in the state of practice
  • Current CPR certification required
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • At least one year of clinical experience, with RN licensure having no restrictions
