General Overview of the Role

The Population Health Pharmacy Technician position entails executing administrative functions related to Medication Management within the Population Health Management Office (PHMO). This role involves supporting Care Managers and clinical pharmacists with medication-related concerns, including medication adherence, access, medication histories, and other responsibilities as assigned by the Pharmacy Technician Supervisor or Assistant Director of Population Health Pharmacy Services.

Key Responsibilities
  • Collaborate with PHMO clinical pharmacists on medication management processes and implement necessary revisions.
  • Facilitate medication reconciliation activities, assist patients in overcoming medication adherence barriers, conduct outreach for medication adherence, and complete referrals to community resources and internal PHMO staff.
  • Gather medication histories, including fill history, for patients with complex medical needs from patients, electronic medical records, and other available resources.
  • Engage with local pharmacies to clarify PHMO programs and resolve issues impacting medication adherence or access.
  • Conduct home visits independently or alongside a population health care manager to collect medication information and provide support to Care Managers and clinical pharmacists.
  • Monitor metrics under the guidance of the Lead Population Health Pharmacy Technician related to the population health pharmacy program, including documenting technician interventions in the electronic medical record.
  • Maintain a foundational understanding of the NC Medicaid PDL, relevant payer formularies, and the prior authorization process for Medicaid and other common payers served by the DukeWELL pharmacy team.
  • Document all patient interactions and activities in Maestro Care or other systems as requested by the Lead Population Health Pharmacy Technician, the Director of Population Health Pharmacy Services, and/or PHMO leadership.
  • Assist with special initiatives and projects within the population health pharmacy as required.
  • Utilize available databases and software to evaluate patient activity and medication adherence, addressing barriers to care when feasible.
  • Participate in staff meetings, in-services, and other program-specific tasks and activities as necessary.
  • Take ownership of personal and professional development.
Minimum Qualifications

Education
  • High school diploma or equivalent and completion of an accredited pharmacy technician training program, or acceptance into pharmacy school, or a high school diploma or equivalent with one year of pharmacy or healthcare-related experience.
Experience
  • At least one year of relevant experience is required.
  • Preferred: A minimum of two years of experience as a Pharmacy Technician.
Degrees, Licensures, Certifications
  • Registration with the North Carolina Board of Pharmacy is mandatory.
  • Certification as a Pharmacy Technician is required from the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
