Job Summary

Duke University is seeking a highly motivated and detail-oriented Pharmacy Technician II to join our team in the Clinical and Patient Care Services division. As a Pharmacy Technician II, you will work collaboratively with the Transitions of Care Manager to perform medication history interviews for patients at Duke University Hospital.

Key Responsibilities
  • Conduct patient interviews to obtain a complete home medication list, including contacting retail pharmacies or physician offices to verify the medication history.
  • Ensure accurate and complete documentation of each identified medication in the electronic medical record.
  • Discuss any identified issues related to the medication history with clinical pharmacist staff to promote high-quality transitions of care.
  • Provide medication history-related training and development to pharmacy technicians and students.
  • Participate in departmental goal planning and quality improvement activities focused on care transitions.
Requirements
  • High school diploma or equivalent and completion of an accredited pharmacy technician-training program.
  • Minimum of one year related experience.
  • North Carolina Board of Pharmacy registration is required.
  • Pharmacy Technician certification is required from the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
