About the Role

We are seeking a highly motivated and detail-oriented Pharmacy Technician II to join our team at Duke University Hospital. As a key member of our Clinical and Patient Care Services division, you will work collaboratively with the Transitions of Care Manager to perform medication history interviews for patients.

Key Responsibilities
  • Use high-level communication skills to perform patient interviews and obtain complete home medication lists.
  • Contact retail pharmacies or physician offices to verify medication history with persistence to achieve the highest level of accuracy.
  • Ensure complete and accurate documentation for each identified medication in the electronic medical record.
  • Discuss any identified issues related to medication history with clinical pharmacist staff to promote high-quality transitions of care.
  • Provide medication history-related training and development for pharmacy technicians and students.
  • Participate in departmental goal planning and quality improvement activities focused on care transitions.
Requirements
  • High school diploma or equivalent and completion of an accredited pharmacy technician-training program.
  • Minimum of one year related experience.
  • North Carolina Board of Pharmacy registration is required.
  • Pharmacy Technician certification is required from the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
