Key Responsibilities:
- Conduct thorough patient assessments and develop personalized care plans
- Implement infection control procedures to maintain a safe and healthy environment
- Collaborate with healthcare professionals in various specialties to ensure comprehensive care
- Administer medications as prescribed and manage patient care
- Assist in case management and patient education to promote optimal health outcomes
Requirements:
- CPR Certification
- LPN License
- Experience in long-term care and rehabilitation
- Ability to work effectively with other disciplines
- Strong understanding of patient care and case management principles
